Diagnosis first: why birthmarks must be classified before they are lasered

With birthmarks, the diagnosis is the treatment plan. Getting the classification right — before anyone fires a laser — decides the outcome.

Two families, two physics

Birthmarks split into two fundamentally different families. Vascular birthmarks (port-wine stains and other red marks) are made of blood vessels — they absorb vascular laser wavelengths, and treatment is a series of sessions that progressively close abnormal vessels; earlier treatment generally does better. Pigmented birthmarks (café-au-lait macules, Becker naevus and other brown lesions) are made of pigment — they need short-pulse pigment lasers at completely different wavelengths and pulse durations. Firing the wrong class of laser at the wrong birthmark achieves nothing at best, and scarring or worsened pigment at worst.

The test patch: how honest birthmark treatment works

For pigmented birthmarks in particular, the correct process is a test patch: a small area within the lesion is treated at defined settings and reviewed at 6–12 weeks. If it clears well with minimal downtime, the whole lesion is treated. The realistic numbers deserve to be said out loud: around 80% of café-au-lait–type lesions respond to short-pulse lasers — and 20% do not, even with the best technology. Some recur after clearance. A plan that doesn't tell you this before you start is not an honest plan.

What doesn't work

Creams and peels do not remove birthmarks. Excision trades a birthmark for a scar and is rarely sensible for large lesions. And "one laser fits all" clinics — where whatever machine was purchased becomes the answer to every mark — are precisely how patients accumulate sessions without results.
